Description
The Licensed Social Worker assists in overseeing the development, implementation, supervision, and ongoing evaluation of social services designed to meet and assist residents in attaining or maintaining their highest practicable well-being. This includes identifying the need for medically-related social services, providing them in accordance with State and Federal regulations, and contributing to comprehensive assessment and goal-setting for assigned residents. The Social Worker encourages resident participation in care plans, facilitates transitions back into the community, and ensures documentation of actions in medical records. They also engage in advance care planning, advocate for residents, investigate alleged abuse, and promote facility policy development.
