FoundCare, Inc. is hiring a full-time Linkage Specialist in Belle Glade, Florida to coordinate HIV prevention, linkage, and support services. The role focuses on linking clients living with HIV and those at risk to medical care, PrEP providers, health insurance enrollment, and community resources, while also handling documentation, outreach, health education, follow-up, and community partnership activities. The position requires social services or related education and/or relevant experience, strong communication and computer skills, a valid driver's license and car, and annual CAC certification; bilingual English/Spanish or Creole and HIV training are preferred.
