Description
The Licensed Practical Nurse provides end-of-life and hospice care to patients in home or community settings, including direct patient care, implementation of approved plans of care, documentation, patient and family education, symptom and pain management, coordination with hospice and interdisciplinary teams, and participation in quality management. The role requires graduation from an accredited practical nurse program, three years of nursing experience, current Georgia LPN licensure, strong communication and organizational skills, and a responsible licensed driver with an insured automobile.
