CircleLink Health is seeking a part-time, remote LPN Care Coach to manage patients enrolled in Medicare’s Chronic Care Management Program. The role involves providing education, coordinating care, closing preventive care gaps, and coaching patients on self-management strategies. Key responsibilities include utilizing specialized care management software, building patient rapport, implementing and improving plans of care, connecting patients with community resources, conducting transitional care management, and closing care gaps. The ideal candidate will possess excellent documentation and time management skills, strong ownership of outcomes, and fluency in both English and Spanish.
