Description
The LPN Care Coordinator assists the Director of Nursing, Assistant Director of Nursing, and RN Nurse Manager in overseeing a nursing unit. Responsibilities include aiding in care planning, evaluating resources, implementing the nursing process, collecting resident data, contributing to assessments like MDS and RAPS, developing care plans, collaborating with interdisciplinary teams, monitoring resident conditions, reporting changes to RNs, participating in care conferences, and ensuring care plans align with physician orders. Additionally, the coordinator provides nursing care under RN supervision, directs LPNs and nursing assistants, supports staffing, ensures safe care delivery, maintains accurate documentation, communicates care needs effectively, adheres to nursing policies and regulations, and supports quality improvement initiatives.
