Benchmark Human Services is hiring an LPN Healthcare Coordinator to provide healthcare coordination, direct intervention, medication and pharmacy oversight, care transitions, documentation, health-risk planning, and training for unlicensed staff for individuals in Supportive Living Homes. The role requires a current Indiana LPN license, a valid driver’s license, and experience in community or long-term care and with intellectual and developmental disabilities or mental illness.
