Description
The LTSS Transition Coordinator supports the LTSS Coordinator or contracted provider in person-centered care transitions for individuals moving between home care, assisted living, and skilled nursing facilities. The role coordinates care plans, waivers, utilization requests, care-team documentation, alternative service levels, critical incidents, and appeals or hearings, while collaborating with healthcare providers, social workers, and other stakeholders. It is primarily remote with occasional in-person meetings, requires a BA/BS and at least two years of relevant experience, and offers a starting salary of $45,000 to $60,000 with health insurance and benefits.
