Description
Vynca is hiring an LVN Case Manager for its Enhanced Care Management team. The role serves as the client’s primary care contact, coordinates providers and benefits, develops care plans, advocates for members, connects clients to social and community services, conducts outreach, documents progress in EMR and CRM systems, and completes reporting. It is a hybrid position requiring travel throughout Humboldt County up to five days per week, with candidates required to reside within 20 miles of the assigned territory. The position requires an active California LVN license, at least two years of relevant care-management or community-health experience, and Monday-Friday availability from 8:30 a.m. to 5:00 p.m.; bilingual English/Spanish ability is preferred.
