Vynca is hiring an Internal Title: Clinical Lead Care Manager, also described as a hybrid LVN Case Manager, for its Enhanced Care Management team in Los Angeles County, California. The role coordinates client care across physical, mental, substance-use, palliative, memory, and social-service needs; develops care plans; advocates with providers; connects clients to community resources; documents progress in EMR and CRM systems; and completes reporting. It is a hybrid position requiring travel throughout the Los Angeles area up to five days per week, with candidates required to reside within 20 miles of the assigned territory. The position requires an active, unrestricted California LVN license, at least two years of relevant care-management or community-health experience, a valid driver's license, reliable transportation, and general computer skills; bilingual English/Spanish ability is highly preferred.
