Vynca is hiring a Clinical Lead Care Manager for its Enhanced Care Management team in Riverside County, California. The role serves as a client’s primary care coordinator, manages care plans and case information, connects members with medical, behavioral health, social, housing, and community services, advocates with providers, and maintains EMR and CRM documentation. It is a hybrid position requiring travel throughout the assigned territory up to five days per week, with candidates required to reside within 20 miles of the territory. The role requires an active California LVN license, at least two years of relevant care-management experience, strong communication and interpersonal skills, a valid driver’s license, reliable transportation, and computer proficiency; bilingual English/Spanish ability is strongly preferred. A $2,500 sign-on bonus is available for offers signed by September 30, 2026.
