Description
The MDS/Care Plan Coordinator ensures timely and accurate resident assessments and interdisciplinary care plans adhere to federal and state guidelines. Responsibilities include identifying resident acuity, determining specific care needs, and communicating these to families and stakeholders. The coordinator integrates various services like nursing, dietary, and physician services to ensure proper assessment and reimbursement, completes Medicare/Medicaid case-mix documents, conducts continuous MDS reviews, and oversees the entire MDS/Prospective Payment System (PPS) documentation and submission.
