Trevida Healthcare is hiring an MDS Coordinator to oversee the RAI process and ensure timely, complete MDS, CAA, and interdisciplinary care plan documentation for residents. The role involves supervising assessment completion and transmission, coordinating care plan conferences, facilitating monthly quality management meetings, and managing the care planning process. Candidates must have a nursing degree, an active RN license (LPN considered), at least 1 year of long-term care MDS coordinator experience, and preferably RAC-CT certification. The position offers a generous compensation package with PTO, paid holidays, medical/dental/vision insurance, supplemental benefits, 401(k), and daily pay.
