Gables Care Center is hiring an MDS Coordinator for its Hopedale, Ohio senior living location. The role focuses on coordinating the Minimum Data Set process, developing and implementing individualized care plans, maintaining accurate resident documentation, and collaborating with interdisciplinary teams to support regulatory compliance and high-quality resident care. The position requires a nursing degree, at least one year of long-term care MDS Coordinator experience, an active RN license (LPN may be considered), and strong communication, organization, problem-solving, EMR, and MDS software skills.
