Batavia Rehabilitation and Nursing is hiring a part-time MDS Coordinator to manage and assess patient care, coordinate the RAI/OBRA/PPS assessment process, maintain MDS and care plan data for electronic submission, review Medicare-related records, and participate in care planning. The role requires a current LPN or RN New York State license, at least two years of long-term care experience, and strong communication and interpersonal skills.
