MDS Coordinator at meadow-wind

Meadow Wind in the Massillon, Ohio area is hiring an experienced MDS Coordinator to manage the RAI process and ensure timely, complete MDS, CAA, and interdisciplinary care plan documentation. The role involves monitoring assessments, supervising MDS input and transmission to the state, facilitating care plan conferences and monthly QM meetings, and managing the care planning process. Candidates must hold an active, unencumbered Ohio RN or LPN license, have a nursing degree, CPR, and at least three years of long-term care experience as an MDS Coordinator; RAC-CT is preferred.
