Experienced MDS Coordinator - LPN role at a faith-based organization focused on resident care, responsible for managing the Minimum Data Set assessment process, accurate documentation, PDPM reimbursement, and regulatory compliance. The coordinator collaborates with interdisciplinary teams to develop person-centered care plans, leads IDT meetings, and uses systems such as PCC, SimpleLTC, NetHealth, Teams, IQIES, Myers & Stauffer, Power DMS, and Google Sheets.
