Platte Health Center Avera is hiring a Patient Care Coordinator in Platte, South Dakota. The role assists the Director of Nursing with department management, supervises and supports RNs, LPNs, and CNAs, covers admissions and rotating nursing shifts, provides resident rounds, coordinates MDS 3.0 and other assessments, maintains care plans, and supports resident safety and compliance activities. The position requires current South Dakota Registered Nurse licensure and prefers clinical and supervisory long-term care experience.
