Description
Buena Vida Rehab and Nursing Center is hiring a full-time MDS Coordinator / MDS Assessor to complete accurate and timely MDS assessments, care area assessments, and care plans in compliance with CMS and New York State regulations. The role requires an active New York State RN license, long-term care or skilled nursing experience, and proficiency with EMR systems such as Point Click Care. Previous MDS experience is preferred, and the position supports reimbursement, Medicare documentation, quality improvement, and communication with the care team and families.
