Majestic Care of Goshen is hiring an MDS Coordinator, RN to supervise and coordinate Minimum Data Sets, care plans, PPS assessments, CATs, and CAAS for residents in a skilled nursing facility. The role includes developing and revising care plans, coordinating care-plan meetings, submitting assessments to state and federal agencies, monitoring resident care, maintaining confidentiality, communicating across departments, and assisting with pre-admission screening. The position requires an active RN or LPN license, MDS 3.0 knowledge, computer skills, and an active CPR license or ability to obtain one within 30 days; long-term care experience is preferred.
