Evergreen Healthcare Group is looking for an experienced Registered Nurse MDS Coordinator to coordinate assessments, care planning, and clinical operations in a skilled nursing setting. The role involves approximately 24 hours per week dedicated to MDS coordination and 16 hours supporting Resident Care Managers. Additional floor coverage will be needed as required.
