Daymark Health is seeking a Medical Assistant to join their care team. This role involves coordinating social and logistical aspects of patient care, connecting with patients and caregivers via various communication channels, navigating community-based services, ensuring follow-up appointments, and supporting seamless coordination with external providers. The Medical Assistant will conduct social needs assessments, develop care plans, troubleshoot technology issues, manage appointments, prepare charts, track surveys, and document interventions.
