Pro Care Innovations is hiring a full-time, remote Medical Assistant - Care Team Coordinator to support its Chronic Care Management program. The role serves as an ongoing patient contact, conducts monthly outreach and care coordination, documents interactions, monitors care plans, coordinates with providers, supports patient retention, and maintains HIPAA-compliant documentation. Applicants need Medical Assistant certification or equivalent healthcare experience; CCM, care coordination, case management, primary care, or population health experience is highly desirable. The position offers a guaranteed minimum pay of $15.00–$17.00 per hour plus productivity incentives, health, dental, vision, retirement, and paid time-off benefits.
