Description
Ohio’s Hospice is hiring a part-time Medical Social Worker Liaison to support hospital admissions and referrals for hospice patients, coordinate care with physicians, discharge planners, social workers, and nursing staff, and facilitate safe transfers to hospice, home, or other care settings. The role provides psychosocial support, education, informed-consent guidance, insurance and financial assessments, advance-directive support, consent assistance, and ongoing social-work case management for patients in facility-based contracted beds. The position is based at Ohio State University Wexner Medical Center and requires travel to Licking Memorial Hospital.
