Description
Proactive MD is hiring a Medical Social Worker, internally called a Patient Advocate, to support continuity of care for employees and dependents at its Santa Fe health center. The role coordinates care with specialists, labs, imaging centers, community resources, and hospitals; assists patients with benefits, prior authorizations, referrals, and navigation; provides behavioral and mental health support; conducts home and site visits; and engages patients before, during, and after visits. The position works Monday through Friday from 8 a.m. to 5 p.m. and requires at least two years of healthcare, medical social work, or case management experience, a bachelor's degree in social work or a related field, healthcare and medical case management knowledge, and HIPAA confidentiality knowledge.
