Ohio’s Hospice is hiring a Medical Social Worker for a PRN position serving patients and families in Wooster. The role provides direct patient care, assesses biopsychosocial, safety, financial, bereavement, and anticipatory grief needs, develops individualized care plans, coordinates hospice and community services, documents care in the electronic health record, and supports interdisciplinary teams and quality improvement. The position requires an on-call rotation, a valid driver’s license, computer documentation skills, and preferably Ohio LISW or LSW licensure and one year of healthcare social work experience.
