Description
The Mgd Care Coordinator manages members’ individual service plans, conducts telephonic or face-to-face needs assessments, identifies potential clinical health-care needs, and coordinates cases with clinical healthcare management and interdisciplinary teams. The role supports non-clinical needs for members with chronic illnesses, co-morbidities, or disabilities, establishes short- and long-term goals with members and their support networks, and identifies members who may benefit from expanded services. It requires a BA/BS degree and at least one year of relevant experience, with a health-care-related field preferred; Community Health Worker certification is required for MMP within one year of employment.
