Ohana Pacific Health is hiring an on-site MDS Coordinator/Licensed Practical Nurse at Garden Isle Rehab & Nursing. The role coordinates interdisciplinary teams and resident assessments, reviews and edits MDS, CAA, care plans, and care conference notes, ensures accurate electronic MDS transmission, and supports reimbursement and resident-centered quality of care. Applicants must graduate from an accredited nursing school, have at least one year of professional nursing experience, hold a current Hawaii Licensed Practical Nurse license, and maintain active RAC-CT certification.
