Description
The MOUD case manager will support people with opioid use disorder through outreach, education, medication navigation, treatment adherence, housing and mental health referrals, and harm-reduction services. The role manages a caseload of 20 to 30 participants, documents client records, supports HEAT and community programs, and may provide jail advocacy in San Francisco and other Bay Area counties. It is a full-time, non-exempt position based in the Tenderloin neighborhood, with a bachelor’s degree in social work, public health, or a related field or two years of relevant MOUD navigation and harm-reduction experience as a minimum qualification.
