Description
The Nurse Case Manager provides field-based, community nursing care to people experiencing homelessness and in supportive housing, including triage, health education, medication administration, chronic and complex care, multidisciplinary coordination, and support for substance use disorder treatment. The role requires an accredited nursing degree, Colorado RN licensure, and Basic Life Support certification, with experience in ambulatory, public, community, mental health, emergency, substance use disorder, or hospice nursing preferred.
