Community Physicians is hiring a full-time Nurse Case Manager in Oakbrook, Illinois, working a Monday-to-Friday 8-hour day shift at $34.00–$36.00 per hour. The role develops and manages individualized care plans, advocates for patients and families, coordinates access to medical providers and community resources, provides health education, assesses unmet needs, supports medication adherence and follow-up, and maintains care-team communication. Required qualifications include an Illinois RN or LPN license, at least three years of nursing experience, two years of geriatric experience, two years of case management or care coordination experience, and post-acute or long-term care experience within the last five years.
