Polaris Group, the SNF Clinical branch of Assembly Health, is hiring a full-time Nurse Consultant to provide nationwide consulting services to long-term care facilities. The role involves clinical and regulatory compliance audits, Medicare and MDS process evaluations, medical-record reviews, mock surveys, documentation and process assessments, and management of assigned facilities for service quality and cost-effectiveness. The consultant must travel to client facilities 75–80% of the time and report to the Consulting Department Manager. The position requires an active Registered Nurse license, five years of long-term care experience, clinical compliance and regulatory oversight experience, and knowledge of Medicare, MDS 3.0, RAI, PDPM, and related regulations. The salary range is $95,000–$105,000 USD, with medical, dental, vision, 401(k), paid holidays, and paid time off.
