Description
The Nurse Navigator serves as the primary clinical resource and care-coordination professional for Independent Living residents, conducting assessments, coordinating care across hospitals, rehabilitation, home health, hospice, and community providers, facilitating safe transitions, removing barriers to services, supporting education and wellness initiatives, and tracking high-risk residents. The role requires a current Registered Nurse license, at least three years of experience working with older adults in a continuing care retirement community, assisted living, nursing home, or hospital, and the ability to travel to partner facilities within a 40-mile radius.
