The Clinical Nurse Navigator/Coordinator provides clinical nursing care and patient navigation in a multidisciplinary clinic setting. Responsibilities include coordinating testing, procedures, follow-up visits, referrals, and provider communications; assessing patients’ physical, psychosocial, and cultural needs; developing and evaluating care plans; delivering education; administering medications; triaging calls and walk-in patients; tracking Pap and mammogram results; managing OB/GYN and fetal monitoring schedules; and supporting quality improvement and staff training. The role requires an associate’s degree or equivalent nursing training, AHA-BLS, fetal monitoring certification or ability to obtain it within 30 days, a current or pending D.C. RN license, and at least three years of registered nurse experience.
