Description
The Registered Nurse supports care coordination for ambulatory medical-group services, including overseeing care-coordination initiatives, facilitating transitions from inpatient and skilled-care settings to home, developing collaborative care plans, providing patient and family education, and supporting adherence to provider recommendations. The role also involves cost-effective problem solving, process improvement, risk assessment, and waste reduction, with requirements for a BSN or equivalent experience, three years of relevant experience, CPR, and Registered Nurse licensure.
