Carease Health - IL is hiring an experienced Nurse Practitioner or Physician Assistant with geriatrics or internal medicine experience to provide preventative care for residents in skilled nursing facilities. The role combines on-site visits with remote work from home and includes use of Remote Patient Monitoring, Chronic Care Management, and Collaborative Care Management, with a focus on individualized care plans, family education, quality improvement, and reducing hospitalizations.
