Adobe Population Health is seeking a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for pediatric and adult members, focusing on health literacy and quality of life. The NP will evaluate medical history, medications, health status, and social determinants of health, coordinating care plans with providers and ensuring accurate documentation. This position requires a minimum of 10 hours per week, including weekdays (3:00 p.m. – 8:00 p.m.) and weekends (8:00 a.m. – 5:00 p.m.), with travel up to 90% throughout designated regions.
