Adobe Population Health (APH) is seeking a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for pediatric and adult members. The NP will evaluate medical history, current medications, health status, and social determinants of health during hour-long visits. Responsibilities include assessing psychosocial and clinical needs, coordinating care plans, supporting healthcare resource utilization, and ensuring timely documentation. The position requires a minimum of 10 hours per week with availability on weekdays (3:00 p.m. – 8:00 p.m.) and weekends (8:00 a.m. – 5:00 p.m.). The role involves traveling up to 90% throughout designated regions to perform assessments, educate members, and provide care coordination, focusing on decreasing medical costs and increasing quality of life.
