Description

AdvisaCare Hospice and Palliative Care Program is seeking a Nurse Practitioner to join their team. The ideal candidate will have current unrestricted Nurse Practitioner certification in Michigan, preferably with one year of experience in a Medicare-certified home health/hospice agency. Responsibilities include conducting medical evaluations, prescribing medications, assisting with care coordination, maintaining accurate patient records, collaborating with interdisciplinary teams, educating patients and caregivers, and supervising nurse trainees.
