Clover Health is looking for compassionate and driven Nurse Practitioners to join their Clover Home Care teams in Mercer, Middlesex, or Somerset County, NJ. The role involves providing comprehensive primary and supportive care directly in the homes of vulnerable members, focusing on longitudinal primary care for those with complex and chronic conditions. Nurse Practitioners will manage a dedicated patient panel, deliver ongoing in-home primary care, and participate in a shared on-call rotation.
