Description
Fountain Life is hiring a Nurse Practitioner to serve as the continuous clinical presence for members throughout a year-long health journey. The role translates diagnostics and physician direction into personalized care, conducts medical assessments, leads follow-ups, manages bioidentical hormone replacement therapy and intravenous and injectable therapies, coordinates with physicians and other care-team members, escalates red flags, documents care, and supports member retention and satisfaction. The position requires a master’s degree in nursing, an active and unrestricted APRN license, ACLS/BLS certification, and proficiency with diagnostics, IV and injectable therapies, and personalized care planning; functional, concierge, or preventive medicine experience is preferred.
