GuideStar Eldercare is hiring a Nurse Practitioner to provide initial evaluations, continuing therapy, cognitive testing, documentation, and multidisciplinary patient care for residents with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. The role involves daily travel from home to assigned facilities, private-practice autonomy, and collaboration with corporate clinical leadership and administrative support.
