GuideStar Eldercare is hiring a Nurse Practitioner to provide dementia and Alzheimer’s care in nursing homes and assisted living facilities. The role involves daily travel from home to assigned facilities, initial evaluations, continuing therapy, cognitive testing, documentation, and professional relationships with facilities, patients, and families. The position requires Nurse Practitioner licensure, interdisciplinary teamwork, autonomy with regional Area Manager support, background and drug screening, and credentialing.
