GuideStar Eldercare is hiring a Nurse Practitioner to provide initial evaluations, continuing therapy, cognitive testing, documentation, and behavior-health-centered patient care for patients with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. The role involves daily travel to assigned facilities, interdisciplinary collaboration, and autonomous practice with regional Area Manager support. Qualifications include Nurse Practitioner licensure, interdisciplinary teamwork, time management, background and drug screening, and credentialing.
