GuideStar Eldercare is hiring a Nurse Practitioner to provide behavior-health-centered care for patients with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. The role involves traveling daily from home to assigned facilities, conducting initial evaluations, continuing therapy, cognitive testing, documentation, and maintaining professional relationships with facilities, patients, and families. The position requires Nurse Practitioner licensure, interdisciplinary teamwork, autonomous practice with regional Area Manager support, credentialing, and background and drug screening.
