GuideStar Eldercare is hiring a Nurse Practitioner to provide initial evaluations, continuing therapy, cognitive testing, documentation, and behavior-health care for patients with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. The role involves daily travel to assigned facilities, interdisciplinary collaboration, and autonomous practice with regional Area Manager support. Applicants must hold Nurse Practitioner licensure, pass background and drug screening, and complete credentialing.
