Conviva Senior Primary Care is hiring a Nurse Practitioner or Physician Assistant to provide comprehensive, value-based primary care to seniors at its Wellington Family Center in Wellington, Florida. The role involves patient care, referrals, hospital and SNF coordination, DME and home health services, documentation, on-call rotation, and growth-plan support. Applicants need a master's degree in nursing or completion of a PA program with board certification, an active unrestricted NP or PA license, and TB screening; experience in value-based care, primary care, or Medicare Advantage is preferred. The position is onsite, full-time, and pays $123,800 to $170,300 per year, with medical, dental, vision, retirement, and other benefits.
