Description
Porter Cares is hiring a Nurse Practitioner to conduct in-home health risk assessments and active medical management for members in Hampden, Hampshire, and Worcester counties. The role includes evaluating medical history, reviewing systems, reconciling medications, taking vital signs, performing physical exams, conducting point-of-care testing, identifying diagnoses, and supporting care management. Applicants must hold an unencumbered Massachusetts Nurse Practitioner license, be licensed as a Medicaid Provider, and have three years of patient care experience; primary care, adult, geriatric, and EMR experience are preferred. Compensation is $110 per completed patient assessment, with a flexible schedule, paid mileage over 20 miles per day, and paid training.
