Segue Health is hiring an experienced Nurse Practitioner to provide in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities to their homes in Denton and surrounding areas. The role includes clinical assessments, discharge-summary review, care-team coordination, consultations, medication prescribing, and ongoing care planning. Candidates must hold a current Texas medical license, active Medicare provider number, Nurse Practitioner certification, and be able to travel within a 35-mile radius of home. The position offers a base salary of $115,000–$135,000, quarterly performance bonuses, and competitive benefits.
