Strive Health is hiring a Clinical Lead Nurse Practitioner to manage a panel of patients with chronic heart failure, chronic kidney disease, end-stage kidney disease, and related conditions. The role provides longitudinal in-person, telehealth, and dialysis-clinic care; coordinates with external providers; manages care plans, documentation, risk adjustment, quality, cost, and outcomes; and supports transitions, shared decision-making, and advanced care planning. Candidates need a nursing master's degree, at least two years of relevant family, primary care, or cardiology/nephrology Nurse Practitioner experience, EHR experience, current licensure and DEA eligibility, BLS or CPR certification, and reliable transportation. The position offers a hybrid-remote arrangement, medical, dental, and vision insurance, and an annual salary of $112,000 to $140,000 plus a potential 10% target bonus.
