Strive Health is hiring a Nurse Practitioner to lead longitudinal, high-touch care for patients with chronic heart failure, chronic kidney disease, end-stage kidney disease, and related conditions. The role manages a patient panel, performs assessments and physical exams, orders and interprets diagnostic tests, coordinates care across home, clinic, telehealth, and dialysis settings, supports care transitions and advanced care planning, and contributes to quality, cost, and outcomes. Candidates need a nursing or similar master's degree, at least two years of relevant family, primary care, or cardiology/nephrology Nurse Practitioner experience, EHR experience, current licensure, national board certification, DEA eligibility, BLS or CPR certification, and reliable transportation. The position offers hybrid-remote flexibility, medical, dental, and vision insurance, and an annual salary of $112,000 to $140,000 with a potential 10% target bonus.
